Vecuronium

Non-depolarizing neuromuscular blocking agent

Mechanism of Action

Competes with acetylcholine for cholinergic receptor sites on the post-junctional neuromuscular membrane, resulting in skeletal muscle paralysis.

Indications

Facilitation of endotracheal intubation (often used for maintenance of paralysis after succinylcholine, or as a primary agent if depolarizing agents are contraindicated); maintenance of paralysis during mechanical ventilation.

Contraindications

Known hypersensitivity to vecuronium or bromides; inability to control the airway or ventilate the patient.

IV, IO

Onset / Duration

Onset: 1 to 3 minutes. Duration: 30 to 45 minutes.

Adverse Effects

Apnea, respiratory depression, prolonged paralysis, bronchospasm (rare), transient tachycardia or hypotension.

Precautions & Interactions

Vecuronium has no effect on consciousness or pain threshold; adequate sedation and analgesia must be administered concurrently. Use with extreme caution in patients with severe hepatic or renal impairment, neuromuscular diseases like myasthenia gravis, or severe obesity.

Special Considerations

Authorized provider levels: RN/Paramedic. Ensure advanced airway equipment, backup supraglottic airways, and bag-valve-mask are immediately available prior to administration. Continuous monitoring of oxygenation, ventilation, and cardiac rhythm is strictly required.
