Ibuprofen

Class

Nonsteroidal anti-inflammatory drug (NSAID), analgesic, antipyretic

Mechanism of Action

Reversibly inhibits cyclooxygenase-1 and 2 (COX-1 and COX-2) enzymes, which decreases the formation of prostaglandin precursors, resulting in reduced inflammation, pain, and fever.

Indications

Mild to moderate pain, fever.

Contraindications

Hypersensitivity to ibuprofen or other NSAIDs. History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs. Active GI bleeding or peptic ulcer disease. Perioperative pain in the setting of coronary artery bypass graft (CABG) surgery. Third trimester of pregnancy.

Onset / Duration

Onset: 30 to 60 minutes. Duration: 4 to 6 hours.

Adverse Effects

Nausea, vomiting, dyspepsia, abdominal pain, GI bleeding, dizziness, headache, renal impairment, increased risk of cardiovascular thrombotic events.

Precautions & Interactions

Use with caution in patients with a history of GI disease, renal impairment, heart failure, hypertension, fluid retention, or dehydration. May decrease platelet aggregation and prolong bleeding time.

Special Considerations

Authorized for EMT, AEMT, RN, and Paramedic. Not recommended for infants under 6 months of age without direct medical control. Ensure adequate patient hydration before administration to protect renal function.
